- Community care team - Neighbor care, not healthcare. Creating a more formal care team, boundaries
- What are our boundaries?
- Also be in the know about services/resources the city offers
- Can we get brochures, documents, etc. from other communities that have implemented care teams?
- Wednesday June 11 ACM - Pineview Reservoir getaway
Continue ACM Planning Meetings twice/month. Call for new facilitators.
